Pets are part of the family, and we want to ensure they will be cared for should we pass before our pet(s). Adopt Me! Bluegrass Pet Rescue created the VIP (Very Important Pet) Legacy Program in an effort to help alleviate some of this worry.  While it is always our preference another family member or friend step in to care for a pet upon an owner's death, we understand that is not always an option. This program allows owners to feel confident they are leaving their pets to a trusted caretaker by designating Adopt Me! Bluegrass Pet Rescue (hereinafter referred to as AMBPR) as a beneficiary in their will.  The rescue will then act as temporary caregiver, providing a loving environment, while working to find the right home (using our “Fit Focus” philosophy) for the companion animal(s). Our goal is to give people peace of mind where their animals are concerned. 

	By enrolling your dog(s) in our program, providing a one-time fee of $100 per animal, and making a planned minimum gift of $5,000 in your estate plan, you have our assurance we will immediately accept your pet(s) into our program upon entering into a hospice program or upon death.
	Up to five (5) dogs may be included, and the agreement will remain in effect during the lifetime of the enrollee.
	AMBPR will provide loving and attentive care, including temporary placement in a foster home, if available, while we search for a suitable adoptive family.
	You may include detailed instructions regarding the care of your pet(s) for both AMBPR and the future adopters.

	Fill out the VIP Legacy Program Enrollment form below.  An Enrollment Form is required for each VIP.  Our Adult Dog Team Manager will follow up for an interview to determine if your pet(s) is/are eligible.
	Add Adopt Me! Bluegrass Pet Rescue to your will or estate planning documents, and name as a beneficiary with a minimum $5,000 gift, to ensure your pets can enter the program. NOTE: A copy must be on file with AMBPR.
	Be sure to inform any necessary parties (executor, family, or friends) of your enrollment so they can contact AMBPR when that time comes.
	In the event of dissolution of AMBPR, initial deposits will be returned to owners.
